Craniofacial Nurse Coordinator to support the Riley Outpatient
Plastic Surgery service line. This is a full-time, fully benefit
eligible role that works Monday-Friday, day shift, with

The Cleft & Craniofacial Nurse Coordinator works with fellow team
coordinators to facilitate the function and efficiency of the team,
ensuring coordinated care for patients and families. This role is
responsible for assisting families in understanding, coordinating,
and implementing treatment plans.
Responsibilities include ensuring that providers and patients are
coordinated appropriately for each cleft and craniofacial team
visit. This position will prepare patient records for each
patient's cleft team visit, which includes requesting reports, test
results, or other pertinent information from community providers to
ensure care is coordinated and comprehensive. On clinic day, this
position will assist with managing clinic flow, answering family
questions, and serving as a resource for fellow team members. The
coordinator will assist with leading the team conference, making
sure to document and understand the recommendations and overall
treatment plan for each patient. This position will follow up on
patients after clinic to review the team's recommendations, ensure
parental understanding of the treatment plan, offer post-clinic
counseling, provide educational information and arrange referrals
as needed.
Other duties include managing incoming clinical phone triage;
creating educational materials for patients and families; and
assisting with coordinating educational opportunities for team
members, families, and the community. This role will be responsible
for helping orient new team members, assisting with data collection
and supporting research projects for the team. The nurse in this
position will act as a role model, resource person and member of
the leadership team, as well as keeping the organization's mission
and vision at the forefront of all patient care and system
activities.
