Job Description Job Description JOB TITLE: Respite Nurse (RN or
LPN) Immediate, Full-Time Respite Nursing care is needed for a
client in Indianapolis. At least 1 year of direct patient nursing
experience and home health agency (or equivalent) experience are
required . Experience with OASIS and NON-OASIS assessments is
desirable. 1 or more years of experience in home health care, wound
care, lab draws, and OASIS assessments is a plus. Requires at least
1 year of experience with each of the following: Vent care G-Tube
care Port Infusion care SERVICE AREA: Indianapolis, Indiana
SCHEDULE TYPE: Patient requires 40-60 hours of care most weeks
(Overtime is paid at 1.5x your standard hourly rate) Additional
hours may also be available, as patient's family (also care
providers to the patient) are expecting another child and will
require time away from the patient for delivery and recovery this
Spring. We'll build a schedule with you and your client to fit your
availability and preferences. EMPLOYMENT TYPE: Full-Time, Long-term
employment No On-Call requirements / No Overnight shifts required.
HCN will expand this role into multiple FT or PT positions if OT is
not desired. Full-Time Nurses receive hourly compensation (paid

agency and serve existing and returning clients in the greater

$40/hour Full-time LPN: $26 - $30 *Starting wage rates are based on
experience, credentials, positive employment history & references,
and overall fit. Job Summary All Respite Nurses perform skilled
nursing functions in accordance with the federal, state, and local
laws and within the guidelines of his/her professional organization
and agency. All functions shall be performed in accordance with the
established policies and practices and state Nurse Practice Act.
The LPN/RN delivers care to clients as delegated by the RN Case
Manager. Reports to: RN Case Manager; Clinical
Administrator/Director of Nursing Qualifications Graduate of an
accredited school of professional nursing. Current license to
practice as a Licensed Practical Nurse or Registered Nurse in
Indiana. Minimum of one (1) year of experience in a home health or
acute care setting or equivalent experience. 1 years of in-home
care experience is preferred. Minimum of 1 year of direct patient
nursing experience is required. Experience with OASIS and NON-OASIS
assessments is desirable. 1 or more years of experience in home
health care, wound care, lab draws, and OASIS assessments is a
plus. Minimum of 1 year of paid experience with each of the
following: Vent care; G-Tube care; Port Infusion care. Active CPR
and First Aid Certifications required (or can be obtained prior to
hire date through our agency or accredited third parties).
Effective written and verbal communication skills. Good
interpersonal communication skills. Current driver's license or
state ID, and reliable transportation. Willingness to travel to
client and/or throughout all stated service area to care for
clients is required. Strong background in nursing, patient care,
and confidentiality. Strong attendance and dependability track
record with previous employers. Desirable character traits:
reliable, honest beyond reproach, professional, compassionate,
detail-oriented (in care documentation), accountable, friendly and
accommodating, respectful, and positive. Familiarity with HIPAA
regulations and medical terminology. Essential Functions/Areas of
Accountability: Provides professional nursing care under the
direction of a Registered Nurse. Communicates appropriate
status/condition changes to the RN per agency guidelines. Provides
skilled nursing visits for clients as directed by the RN Case
Manager. Follows the care plan established by the Registered Nurse.
Recognizes and reports changes in client condition to the Case
Manager and physician, as directed. Communicates information on a
timely basis, consistent with agency policy and urgency of the
situation. Communication is verbal, as needed, and through complete
and accurate documentation in the clinical record. Scope of
clinical practice includes personal care services, assessment of
findings, medication monitoring and administration, client
teaching, emergency intervention, and nursing services within
professional scope of practice and as delegated by the Case
Manager. Individuals with the license designation of "LPN M-IV" may
perform venipuncture for the purpose of obtaining blood samples for
laboratory analysis. Promotes personal safety and a safe
environment for clients and coworkers. Demonstrates knowledge of
safety/infection control practices by compliance with policies and
procedures. Recognizes and responds appropriately to potentially
unsafe situations. Demonstrates safe practice in the use of
equipment. Makes recommendations on improvement of safety. Performs
job in compliance with agency policies and procedures and
professional and community standards. Accepts responsibility for
professional nursing role. Follows applicable state and federal
laws and agency standards. Attends all meetings as required.
Maintains confidentiality in all aspects of the job. Refrains from
discussing client or staff identities, conditions, or circumstances
in public or with others who do not have a legitimate interest in
the person. Participates in the development, implementation, and
evaluation of the Agency Quality Improvement Program and pertinent
activities. Performs other related duties and responsibilities as
