during the hiring process. How you’ll make an impact in this role -
Lead the interdisciplinary care strategy from admission to
discharge to ensure every patient experiences a seamless, safe, and
well-coordinated transition home or to their next level of care. -
Advocate for patient needs and personal values by removing
financial or logistical barriers, ensuring they receive the
necessary resources and support for a successful recovery. -
Evaluate medical necessity and care levels continuously to ensure
patients receive the right treatment at the right time while
optimizing hospital and insurance resources. - Collaborate with
families, clinical teams, and insurance providers to build a
unified plan that prioritizes high quality outcomes and prevents
gaps in post discharge care. What minimum requirements you’ll need
Licensure / Certification / Registration: - Registered Nurse
obtained prior to hire date or job transfer date required. -
Certified Case Manager credentialed from the Commission for Case
Manager Certification (CCMC) preferred. ACM also accepted.
Education: - Diploma from an accredited school/college of nursing
OR Required professional licensure at time of hire. What additional
preferences we're seeking - At least 2 years of recent acute
inpatient Case Management experience and 3 years of acute inpatient
