the lives of others.Position Summary:
- The Immunization Coordinator works with the clinical team to
meet or exceed national benchmarks for immunization rates for
OneWorld pediatric patients. The Immunization Coordinator is
responsible for managing and maintaining clinic vaccine
inventory.
- JOB-SPECIFIC DUTIES & RESPONSIBILITIES
- 1. Initiates interventions to prevent individual pediatric
patients from experiencing vaccine delays; brings those with
delayed immunizations up-to-date.
- 2. Analyzes data and implements strategies to ensure that the
organization is meeting goals for the Uniform Data System (UDS)
reporting requirements for childhood immunizations.
- 3. Analyzes data to determine trends and areas of opportunity
to build effective strategies for patient outreach and call
backs.
- 4. Uses available resources to retrieve and data enter
historical immunization information for individual patients in the
Electronic Health Record (EHR).
- 5. Monitors and gives feedback to clinical staff to ensure that
vaccines are charted in the EHR in an accurate and timely
fashion.
- 6. Contacts parents/guardians to schedule appointments for
immunizations and/or Well Child Checks.
- 7. Documents call-back/mail efforts in the EHR.
- 8. Meets or exceeds monthly goals for immunization case
management activities.
- 9. Manages inventory, ordering, receiving, shipping, stocking,
labeling, and storage of clinic vaccines in strict accordance with
the Vaccines for Children Program requirements (VFC); including
making reports and resolving reporting discrepancies.
- 10. Manages daily refrigerator temperature logs and addresses
alarms.
- 11.Tracks and communicates immunization recommendations to the
clinical staff.
- 12. Participates in the Vaccine for Children's Program (VFC)
compliance site visits (including unannounced visits) as well as
other educational opportunities associated with VFC program
requirements.
- 13. Assists in the orientation of new staff as needed; provides
nesiis vaccine training/support for clinical staff.1 . Consistently
follows through to accomplish job functions and/or meet
deadlines.2. Demonstrates ability to be flexible and adaptable to
changing work conditions and/or project requirements.3. Exhibits a
positive attitude and good work ethic; works well with others.4.
Provides excellent customer service, both internally and

Other duties as assigned.QUALIFICATIONS1. High school diploma or
equivalent required.2. Medication Aide Certificate preferred.3. One
or more years of work experience in a healthcare environment
preferred.4. Proficiency in both English and Spanish (reading,
writing, and speaking) required.5. Familiarity with the CDC
guidelines for childhood immunizations preferred.6. Proficient
computer skills including alpha and numeric data entry as well as
Microsoft Office products (Excel, Word, Outlook) required.7.
Demonstrated ability to communicate effectively with providers,
supervisors, and support staff to ensure that work duties are
carried out.8. Demonstrated ability and commitment to continually
improve performance through reevaluating procedures and increasing
proficiency.9. Demonstrated skills in time management, task
orientation, analysis, and detail orientation.
Requirements:
