• Certification Details
• BLS - Must be AHA
• Nursys - Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs
• License/Certificate Verification - Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals
• MMR - Vaccines or Positive IGG Titer required
• Fit Test - Required for all disciplines, Annual
• Hepatitis B - Documentation of vaccination series completion OR documentation of declination to vaccine
• Influenza Vaccine - Updated every year
• 2 Step PPD OR IGRA (Quantiferon/Tspot) - 1st within 1 year, 2nd within 30 days of start
• Varicella - Documented proof of two vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles)
• Tdap - Valid 10 years from the administered date
• Ascension Criminal Background Check Results - Include various checks and updated annually
• Ascension Skills Checklist - Clinical only; Required annually in specialty
• Ascension Pharmacology Exam - Pharmacology 1,2 exam required
• Ascension Medical Surgical Exam - Completed prior to start
• Ascension Color Vision - Updated every 3 years
• Ascension Core Competencies - HIPAA, Joint Commission -NPSG, OSHA Topics
